Fortive will match 100% of each dollar you contribute (pretax, Roth after-tax, or a combination of pretax and Roth after-tax contributions) on the first 3%, plus 50% of the next 2% of eligible pay that you contribute each pay date. After one year of service, you may be eligible to receive an additional 2% Company Retirement Contribution equal to 2% of eligible pay each pay date. Remember: You are 100% vested in your own contributions to the Plan.

Basic Life Insurance:

As a Fortive employee you are provided Basic Life Insurance with a benefit in the amount of 1.5x annual pay to a maximum of $2,000,000. This benefit is provided and paid for by Fortive at no cost to you. An optional life insurance plan is also available for purchase.

Optional Life Insurance:

You may purchase optional life insurance on yourself in the amount of one half of your annual pay to a maximum of eight times of your annual pay (or coverage that does not exceed $3,000,000 when basic and optional life are combined).

Spouse Life Insurance:

You may choose voluntary life insurance for your spouse in the amount of $10,000, $25,000, $50,000, $75,000, $100,000, $150,000, $200,000, and $250,000.

Will Preparation:

Securian Life provides free and simple will preparation for you and your spouse through a legal services arrangement with Ceridian.

Child Life Insurance:

You may choose voluntary life insurance for your child(ren) in the amount of $10,000. No matter how many children you have, each eligible child is covered at $10,000.

Voluntary Insurance Benefits
As a Fortive employee you have the opportunity to purchase additional insurance coverage to help offset medical costs, including Group Accident Insurance, Group Critical Illness Insurance, and Group Hospital Indemnity Insurance.

Group Accident Insurance:

Fortive offers Voluntary Accident Insurance through Unum.  When you or your family members receive treatment after a covered accident, the last thing you want to worry about is how out-of-pocket expenses associated with treatment are adding up. Unum Accident Insurance can provide benefits paid directly to you for:
 Urgent care and emergency room visits
 Ambulance and other transportation to the hospital
 Initial care and surgery
 Hospital stays and lodging expenses related to the accident
 Follow-up care, such as doctor’s office visits and physical therapy appointments

Coverage is available for you, your spouse and your dependents, Unum Accident Insurance can be there for you during challenging times.

Group Critical Illness Insurance:

Fortive offers Voluntary Critical illness coverage through Unum.  Critical Illness coverage can provide financial assistance when a serious illness strikes. Benefits paid by Critical Illness Insurance policies can help you with deductibles, co-pays and other out-of-pocket costs when you are diagnosed with a covered critical illness, such as a heart attack, stroke or cancer.

Group Hospital Indemnity:

Fortive offers Voluntary Hospitalization Insurance through Unum.  Hospital Insurance helps protect your finances when there’s an emergency. When you or a covered family member is admitted to the hospital, these policies can pay a benefit directly to you to use as you see fit, whether to cover immediate medical costs and travel expenses, or to help keep the bills covered at home. Whether you’re being treated on an inpatient or outpatient basis, this coverage can help you manage your expenses during a stressful time.
